What is a d4 medical test?
A doctor will need to complete this form if you are applying for a lorry or bus driving licence.

What does a medical involve for HGV?
A physician will examine the HGV student and ask questions relating to things such as epilepsy, seizures, blackouts, memory problems, Parkinson’s or Alzheimer’s disease, narcolepsy and cataplexy, stroke, prior brain surgery, or any chronic neurological condition such as multiple sclerosis.
Do I have to send my old driving Licence back to DVLA?
You must send your old photocard licence to DVLA when you get your new licence. You’ll be told the address to use when you finish the application.
Can I use tippex on a DVLA form?
Complete the form in BLACK ink only – any other colour ink will cause your form to be rejected. Do NOT use correction fluid – this will cause your form to be rejected.

Can you drive while waiting for a replacement license UK?
If your entitlement has expired, you can drive as soon as the DVLA receives your correct and complete application for a new licence as long as you meet the Section 88 criteria. You must only drive vehicles you have applied for on your current application and were entitled to drive on your previous licence.

As of 20 June 2019, DVLA no longer accepts medical applications (D4) completed by Doctors on Wheels Ltd. You can’t download the D2 application form because it includes features that can’t be printed. You have to order a printed copy from DVLA.
Can you print DVLA forms?
Yes – but DVLA forms download options are limited to those applications which do not require a window for your photograph to be supplied. This means that application guides for certain forms, along with DVLA forms v62, V317 and V890, are available to download and print.
